Skilled Nursing Care: Registered Nurses (RNs) & Licensed Practical Nurses (LPNs)
Registered nurses provide a broad range of nursing services based on a thorough assessment of the client's health care needs and development of an individualized care plan. Examples of services provided include skin care, medication management, diet, nutrition management, and care focused on a particular diagnosis. Physician orders and efficient care coordination are integral aspects of nursing services. Licensed Practical Nurses assist with rehabilitation, dressing changes, oxygen administration and medications, and offer services as catheter care under the supervision of an RN.
Pediatric Nursing (PNs)
Pediatric Nursing is a high tech specialty nursing care designed especially for our pediatric patients. By fulfilling the unique needs of our youngest patients with unsurpassed skill and compassion, we have set a new standard for pediatric homecare. Regardless of prognosis, we remain strongly committed to helping families bring their medically complex children home to familiar surroundings. Care can include IV therapies, central line IV care, peripheral IV insertion and care, premature infant care, synagis administration, anticipatory guidance, maternal-child care, hyperbilirubinemia care,
enteral nutrition, oxygen therapy, tracheotomy care, ostomy care and wound care.
Physical Therapy (PTs)
Full-time Physical Therapist - Home Health. Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician's plan of treatment, taking into account individual patients' special or age related needs.
Occupational Therapists (OTs)
Occupational Therapists perform therapeutic exercises and instruct in activities such as meal preparation, toileting, and grooming. OTs also assist with energy and endurance problems, and address equipment needs such as grab bars. Self-reliance is always a goal.
Speech / Language Pathologists (SLPs)
Speech / Language Pathologists provide treatments to improve speech, swallowing, communication, and problem-solving skills. Often applied with other therapies, speech therapy significantly contributes to stabilization and recovery.
Medical Social Workers (MSWs)
Medical Social Workers assist with community resource information for needs such as housing, finances, transportation, caregiver respite, and the need for additional care. MSWs also provide assessment and counseling directed toward improvement of coping skills, stress reduction, and depression/anxiety.
Infusion Therapy
Serving adults and children alike, Human Touch's home-delivered infusion drug therapies include:
- Antibiotics
- Enteral nutrition
- Hydration
- Pain management
- Cancer chemotherapy
Home Health Aides or Certified Nursing Assistants (HHAs) or (CNAs)
In addition to the services provided by homemakers, assist with "hands-on" personal care needs such as bathing, dressing, walking, and toileting. HHAs or CNAs cannot give injections or dispense medications, though they can remind the patient to take their medication.
Homemakers or Companions
Homemakers or Companions can provide assistance with meal preparation, light housekeeping, laundry, errands, and other "hands-off" tasks. Some home care agencies offer these services in addition to more complex services - look for the @ next to the agency name below. There are also specialized agencies that provide only companion-level services - see last page.
Live-Ins
A live-in caregiver is a cost effective way, when needed, to insure the safety and well being of a loved one in their home. Home health caregivers live in the home on a full time basis, helping the client maintain normal routines as much as possible, while providing the necessary care and companionship. When suitable, Live-ins are the most economical solution where many hours of care are required.
Durable Medical Equipment (DME)
Human Touch offers a huge array of modern, state of art medical equipment. Our product line includes: Ambulatory aids, such as wheelchairs and scooters. Patient room equipment, such as hospital beds and commodes. Bathroom safety items, including bathtub safety rails and transfer benches. And much,much more to choose from.
